Epidemiology:

  • Most common medical complication of Pregnancy
  • affects 2-5% of pregnancies
    • Gestational DM 90%
    • Preexisting DM 10%
    • Risk of fetal loss
    • perinatal mortality 48/1000 for babies of diabetic mothers, compared to 8.9/1000 for background population

Maternal Hyperglycemia:

  • Causes fetal hyperglycemia
  • Leading to fetal hyperinsulinaemia
  • Fetal hyperinsulinaemia - even short periods (1-2hours) lead to detrimental consequences in:
    • fetal growth or fetal well-being

Fetal surveillance:

  • screening for congenital anomalies
  • monitoring for fetal well-being,
  • ultrasound assessment for estimated fetal weight and macrosomia.
